3.2设计问题
　　脚手架和模架倒塌的主要原因是支撑失稳,由于许多施工企业在模板工程施工前，没有进行模板设计和刚度验算，只靠经验来进行支撑系统布置，使支撑系统的刚度和稳定性考虑不足。另外，目前在模板支撑系统或脚手架设计计算时，其计算简图采用钢结构的铰接节点，各杆件交于一点，而钢管搭设是用扣件连接，钢管受力是偏心荷载。因此，现场实际情况与设计计算有相当大的差距。还有的钢管材料锈蚀或磨损严重，有的局部弯曲或开焊等，使钢管实际受载能力减少很多，在现场管理不严的情况下，极易发生模板支撑失稳现象。
3.3施工问题
　　目前有不少施工工地的技术负责人，没有对操作工人进行详细的安全技术交底，加上有些工人素质较差，难免发生应用问题。如有的模架倒塌事故是由于操作工人没有按设计要求设置剪力撑或纵横水平拉杆的间距，造成模架稳定性不足；有的事故是工人私自拆除外脚手架与建筑物之间的连接拉杆，导致脚手架整体倒塌；还有的事故是在脚手架和模架上集中堆放建筑材料、预制构件或施工设备等，造成局部杆件超载失稳，引起整体倒塌。因此，施工现场管理混乱，操作人员没有严格按设计要求安装和拆除支撑，也是造成倒塌事故的重要原因。江苏南通如皋市脚手架厂   刘经理  章经理
